I was using New Focus Pico motors, not MDrive hardware. If I exited the EPICS ioc-shell using the command, "exit", the problem appeared. This would lead me to start my search for code that has hooked into the iocsh exit routines (atexit ). Note: things appeared to shutdown cleanly if I exited with a ctrl-c.
