Subject:	bug in VXI resource manager

Hello all,

Thanks to Ric Claus of SLAC a bug has been isolated in the VXI
resource manager.

Symptiom:
correct slot isnt located when EPICS_VXI_LA_BASE is set to
something other than zero.

I have applied a fix to epics_R3_12_2_patches and 
epics_R3_13_0_alpha4 in CVS at APS.
