I'm sure you are all tired of this thread, but the exact
symptoms we had are the following: 
We logged onto an ioc with telnet.
We killed and restarted the sequencer as many times as we wanted to in
the same telnet session,
but when we logged off of telnet, the ioc died.
We would never see a problem if we had a serial interface to the ioc.
This problem went away when we removed the line that closed the log file.
