I have just put a Complete example build for any IOC Blade 9010 fitted
with PCM-3370 PC104+. 

Simply down load and unzip under root to replace your existing backed up
directory.  Includes simple example EPICS drivers for the Blade for the
following IP Cards....

8411
8505
8506
8512
8513
8601
