Sure, run time failure needs to be considered. Falling back from PTHREAD_PRIO_INHERIT to PTHREAD_PRIO_NONE is relatively safe. What we really need is recursive mutexes. Removing the pre 1995 compatible code (older than VxWorks 5.5, yay) would simplify the implementation a lot.
