As of late last week there are two PINI states called RUNNING and PAUSED which
cause processing at the very end of the iocRun and iocPause commands
respectively, so RUNNING does occur after both dbCa and rsrv have started.
There is no guarantee that all dbCa links have connected up by then though,
and I've not checked whether the local channels have although I suspect they
might — it probably depends on task priorities so might be different on
vxWorks vs Linux.
