There have been a number of changes to the environment parameters since
section 7.8 was last revised.
There needs to be documentation on the osiClockTime and osiNTPTime time
providers, and the use of the EPICS_TIMEZONE parameter on vxWorks, and
how this works on RTEMS.

Bug description:
