I melted a vacuum valve when the interlock did not test that the valve was Fully Openned and failed to test that it was also NOT FULLY Closed. When the intermediate electronics were unplugged - both the open and closed limit switch were true. It looked a lot like this one. Turns out that was due to a piece of debris in the valve air solenoid, not a logic problem. Though if the beam had dumped as soon as the fault was detected it probably would have been OK.
