Two answers.
First, the old "rsrv" CA server implements all of the EPICS_CAS_XXX based
configuration _except_ EPICS_CAS_INTF_ADDR_LIST and
EPICS_CAS_IGNORE_ADDR_LIST.
Second, the R3.14 IOC is still based on the old "rsrv" CA server.
