Hi Geyang

Try setting up the environment variable QT_PLUGIN_PATH to include the location of where the Epics Qt libraries are located,

Note: on Linux the path needs to be set to include the location a directory called "designer" which contains the shared,
libraies - this may be the case for Wiondows as well.
