We replaced the PLC5 versions for our TR13 cyclotron with Ethernet capable ones, PLC5/20E and PLC5/40E. This allowed us to transition from a DH+ based console to EPICS without cutting the cord. I used John Sinclair's driver from SNS but heavily modified it to handle the memory arrangement in the PLC,as the SNS driver assumes a contiguous block of data transfer memory, and the TR13 used randomly scattered bits in many words. We could not change the PLC code due to licensing. I can provide a tarball.

Mike, We are using OPC server with RSLinx Gateway to comm PLC5 with the IOC. The guy who did that recently left but I could dig up some details if you want to pursue that.
