Actually removing -D is an oversight on my part, it should still be an option, although
most usage won't be necessary with the addition of epicsGetExecDir() to reliably find
../../dbd/softIoc.dbd at runtime.

The version in the pva2pva module lags behind the one in Base.  It will catch up
the next time I work on that module.  This shouldn't be a big job if someone else
wants to have a go before then.
