Thank you all for the responses...

Assuming that we've been running this same software
for sometime... could hardware failures cause these
symptoms, i.e. cards going bad, flaky power supply?

Also what version of VxWorks would you recommend
running with R3.13.9?

I see that the very latest version of R3.13 does have the warning message
"CAC block sem corrupted" which is nearly identical to the one that you are
reporting. The message occurs when the vxWorks semGive() function returns an
error. My best guess is that some agent is corrupting the client library's
data structures.

Perhaps the board fail status may be occurring because of an exception at
interrupt level (sometimes because the interrupt dispatch table has been
corrupted).

The generally accepted solution is to add more mbufs, and this seems to have
been sufficient at many sites, but you should know that mbuf starvation
issues are occurring more frequently at the SNS lately and therefore are
receiving heightened attention.

A fix was made in R3.13.9 to make the software more robust in low mbuf
situations, but this was fixing a rarely occurring bug with different
symptoms than what you are reporting.

Many patches have been applied to R3.13 since this release.
