default.dctsdr => all ascii file

This morning I successfully ran a ioc that loaded all menu, record, etc
definitions via ascii files rather than using default.dctsdr.
Since a LOT of code had to be changed, added, and thrown away in
order to reach this point this was a rather major milestone.
Of course I still have a lot of testing to perform and also have
to update documentation.

At the upcoming EPICS meeting I will give an overview of the new system.
After release 3.12.2 becomes official (soon) I will fold all my
changes into the development tree at APS. I will then welcome some
beta testers. This should happen about mid November.
