Please check something on the board first:  I believe the STR7201 has switches
to enable A32, A24 and A16 address modes, and the factory configuration is to
set them all to ENABLED.  Make sure you disable A24 and A16.  Are you using
the STR7201 from Struck, or the SIS3801 from SIS, which is essentially an
identical model, but with some additional features?

I have several of the SIS3801 boards, but so far I have been using them with
MVME167 CPUS.  I do have an IOC with the same setup you have (mvme162 16MB,
EPICS R3. 13.1.1, vxWorks 5.3.1), so I will try it today and let you know.  I
can also borrow a real STR7201 to test that as well.

Note that I have updated devSTR7201.c and drvSTR7201 to take advantage of the
new features of the SIS38xx boards, namely the on-board 10 MHz clock and the
prescaler.  With the SIS38xx you can select internal or external channel
advance and a prescaler to be applied to either channel advance input.
