Have you solved this yet?  If not there is one step that you might have
missed:

The first 8 bytes of the downloaded image may have to be set manually
before blowing the boot into flash - use the 162-Bug command MDS <address>
to look at the start of the image.  The first 4 bytes should contain the
initial stack pointer (probably 00 10 00 00), and the next 4 a pointer to
the location of the first instruction to be executed on power-up (ff 80 00
08).  These may not be in the downloaded image, in which case you should
insert them before the first byte that was downloaded, and then program
the whole image starting with those bytes into flash.  I have full
instructions for our use here at
http://www.aps.anl.gov/asd/controls/vxWorks/boot172.html - they should
still apply, although they're for a slightly different board and the IP
addresses will obviously need adjusting.
