My thanks to Andrew and Steve for replying.
It appears that I did not do the gmake from the <top> directory. I was
missing the O.linux-x86 in top/configure which contains
CONFIG_APP_INCLUDE. The release includes are configured from here.

It is by default on all C and C++ compilations, assuming you're properly
using the usual EPICS build rules (see the Makefile in the MakeBaseApp
exampleApp/src directory for the two essential include lines).
Note that the *order* of the -I options given to the compiler matches
the order of the lines in your configure/RELEASE file, where EPICS_BASE
should usually be placed last.
