I made a software component specification template that uses UML diagrams.
Such documents aims at specifing Epics Databases, this is not about the modeling of the Epics Base so this is not
exactly what you asked for.

I am interested in modelling at and above the Record class, to describe
the specialisation of Record into other types, such as ai, mbbi,
waveform, etc. I intend to use the UML model to design and document some
changes to our vacuum control systems, which seem to be increasing in
complexity. Your model would make a well defined package as a basis for
the underlying EPICS activities.

Does anyone have a UML model of EPICS to hand (partial or otherwise)? At
the moment, I could do with just the package/class diagrams.
