Quick follow up on this…

The problem turned out to be this line in /etc/rc.local:

chmod a+rw /dev/video*

It wasn't being run, even though the other lines in that file were. So if I run that command manually after a reboot, the camera works fine.

Nothing to do with RHEL6 kernel version number (I tried RHEL6.4 and RHEL6.5), or the fact that the server had a RAID card in it.
