I can fix this by switching dbProcessNotifyExit() and dbChannelExit()
from separate atexit callbacks to being called (conditionally) from the
exitDatabase() callback.  This fixes the crash Ben found.

The remaining atexit callbacks are:

> atExit dbBkptExit((nil))
> atExit exitDatabase((nil))
> atExit dbLockExit((nil))
> atExit ca_client_exit_handler((nil))
> atExit dbCaExit((nil))
> atExit cacExitHandler((nil))
> atExit twdShutdown((nil))
> atExit errlogExitHandler((nil))
> atExit ClockTime_Shutdown((nil))

From a quick look I would guess that at least dbBkptExit(),
dbLockExit(), and errlogExitHandler() can cause similar problems.  So
I'll look at making these conditional as well.
