casCtx.h and caHdrLargeArray.h have an odd status. They have never been
considered public API, and are thus not installed. However, cagateway
has long used them.
For the debian packages, my solution was to add a special package for just these
headers, which is only needed when building cagateway.

CA gateway build complained about missing <casCtx.h> which I could not find in the package. I build base 3.15.6 and base 3.14.12.3 with INSTALL_LOCATION and the file was missing, built them both without it and the file was there (obviously). So the question is, is it possible to add the portable CA server (PCAS) library source files to INSTALL_LOCATION?
