It might help to see these log lines from the mgmt's arch.log. Specifically, we should see something like so

2    [main] INFO  config.org.epics.archiverappliance.config.DefaultConfigService  -
2    [main] INFO  org.epics.archiverappliance.config.DefaultConfigService  - Obtained my identity from environment variable test-arch
2    [main] INFO  org.epics.archiverappliance.config.DefaultConfigService  - My identity is test-arch
3    [main] INFO  org.epics.archiverappliance.config.AppliancesList  - appliances.xml file specified in the environment as /scratch/.../single_VM_appliances.xml
11   [main] INFO  config.org.epics.archiverappliance.config.DefaultConfigService  - My identity is test-arch and my mgmt URL is http://test-arch.slac.stanford.edu:17665/mgmt/bpl

The identity, "Obtained my identity from environment variable" comes from the environment variable ARCHAPPL_MYIDENTITY. This should match an appliance definition in the appliances.xml "appliances.xml file specified in the environment as /scratch/.../single_VM_appliances.xml" specified using the environment variable ARCHAPPL_APPLIANCES.
