Cannot reproduce.
Tried this record with both R3.14.12.6 and R7.0.2.2 softIoc:

record( bi, "bandgap1")
{
   field( ZNAM, "disabled" )
   field( ONAM, "active" )
}


Adding a 'Check Box' widget, enter bandgap1 as PV name, leaving rest on default, and it simply works.
