Thank you very much gentlemen. Those two patches fixed the problem nicely.
With patched SynApps 5.7 (asyn-4.21, stream-2.6), asynRecord standalone or a set of DTYP=stream records in my IOC are able to connect back if underlying usb-serial device is unplugged from and plugged back to system.
If the usb-serial device is unplugged (physically disconnected), asynReport() command reports disconnected state correctly.
Regarding udev rules (Linux OS), it seems that asyn serial driver has no problem with symbolic-link linked devices which can disappear from /dev/ directory. So I removed my custom udev rule and reverted my st.cmd pointing back to particular /dev/serial/by-id/ file which always points to actual ttyUSB0,1,....
e.g.: drvAsynSerialPortConfigure ("PORT","/dev/serial/by-id/usb-Prolific_Technology_Inc._USB-Serial_Controller_D-if00-port0", 0, 0, 0)
However it might be more convenient or safe to have a specific /dev/ device using udev rules with:
OPTIONS="ignore_remove", NAME+="ttyUSBmyDeviceX"

I have fixed the problem. It turns out that there were bugs both in asyn and in StreamDevice.
The bug in asyn was that it was not disconnecting the port when it detected errors in calls to fcntl and tcsetattr. Once this was fixed the asyn port correctly went into the "disconnected" state when the USB serial device was removed.
However, this did not allow correct functioning, due to a bug in StreamDevice. This bug caused StreamDevice to get into a tight infinite loop in WriteHandler if pasynOctet->read() kept returning asynError, which it does when the device is disconnected.
This is the fix for drvAsynSerialPort.c (patch also attached)

With these 2 patches it behaves correctly. When the USB device is removed the port becomes disconnected, and StreamDevice prints an error message.
When the device is reconnected it continues normally.
I did my testing with a Newport CONEX-AGP motor controller. I created the following udev rule:

This does 2 important things:
- Makes the device always have the same name when it is connected (ttyUSBAGP). Otherwise its name can change from ttyUSB0 to ttyUSB1, etc. each time it is connected, and then it won't work.
- Gives the device world r/w permission.
I have committed the asyn changes to SVN. Once Mazanec verifies that it fixes his problem I will release asyn R4-23.
I have also committed the StreamDevice change to the APS BCDA SVN repository, but that is just a local fix.

I've tried to distinguish stream and asyn behaviour.
With my IOC compiled with both asyn & stream with SynApps 5.7 and no DTYP=stream records loaded, asynRecord is not able to disconnect the PORT. Neither Asyn , if device is physically disconnected.
This might not introduce any change to IOC setup.
However, if the IOC is compiled with asyn only and still the only loaded record is asynRecord. Disconnection using PCNCT and CNCT works fine, no matter if the device is physically disconnected.
After I did read your post, I've tried to set TMOT to zero and repeated the second case (w/o stream).
Regardless that read is almost imposible with zero time-out, disconnection didn't work properly -- asynReport still showed "Yes" to connected state of the PORT even when physical device disconnected. Flush didn;t help neither.

Eric reproduced the problem yesterday, and we think we understand the problem. It appears to actually be a problem with asyn, not with StreamDevice.
The problem does not occur for normal read operationgs with a non-zero timeout. In that case if read() returns an error indicating that the device is no longer available then the driver disconnects the port. For the next read or write operation asynManager will see that the port is disconnected, and will try the normal autoconnect mechanism. When the device available again it will connect.
The problem is occurring when StreamDevice calls read with a zero timeout, which it does to simulate a flush() operation. In this case the driver is calling fcntl() which is returning an error. It then returns that error to StreamDevice. But it does not disconnect the port when this occurs, like it does when a read() error occurs. It should be disconnecting the port. There may be a few other places where errors should cause the port to disconnect.
As a side note, why is StreamDevice calling read() with zero timeout, rather than calling flush()?
I think the fix should be pretty easy. I will try to reproduce the problem today, and then verify that it is fixed.

I will have a look. In older versions of StreamDevice, I check the
connection state at each access and try to connect if necessary. But I
have been told that this is wrong behavior and I should better leave it
to asyn to do the auto connect. So I removed the connect code from
StreamDevice. This worked fine with sockets. But I have to investigate
what is missing with USB to serial. I also check with Mark and Eric. But
it may take a while...
