Where is this documented?  The 3.15.7 and 3.16.2 Application Developer's Guide only says this about PINI:

7.4.10 Initial Processing

initialProcess processes all records that have PINI set to YES. initHookAfterInitialProcess is announced.

What is the difference between PINI=YES and PINI=RUNNING?  Is there anything in that menu that happens later than YES during iocInit?

When an IOC boots, it starts its components in a specific order.
At the time when PINI=YES records are being processed, Channel Access is simply not fully up yet.

Note that for a while PINI has been a menu with different options. Maybe you can get your record processed in a later stage that way.
